7th Grade Vaccination Requirements

According to Georgia Law, all children born on or after January 1, 2002 who are are attending 7th grade, and those who are enrolling into a Georgia school in gradess 8 through 12, must have received one dose of Tdap vaccine and one dose of meningococcal conjugate vaccine.

Contact your healthcare provider or the Henry County Health Department to receive any vaccinations and to obtain the Georgia Certificate of Immunization (Form 3231)
