Administration of Medicine At School

Attention SMS Parents/Guardians, 

If your child needs to take any medication throughout the school day, we can not administer any medication without a Medication Authorization Form signed by a parent/guardian of the student. Once the signed form is turned into the Clinic along with the medication, your child can begin taking the necessary medicine at school when needed or as prescribed by a healthcare provider.

Please do not send medications to school with the student to take throughout the day without written authorization. If so, it will be confiscated.

If your child gets sick in the middle of the school day, SMS Clinic Aid or designated staff CAN NOT provide them with any medicine, but you are free to bring conveniently appropriate medication for your child.
