REGISTRATION REQUIREMENTS  

Proof of Residency: 

In order to enroll, the enrolling parent must have the following documents in their name:

1. Valid Lease or Rental agreement with a parent as leaseholder and signed by both parties AND a GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.

 OR

2.  Property Tax Bill or Settlement Statement in parent’s name AND a GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.

OR 

3.  An affidavit of residency – affidavits of residency are obtained through the County’s  Welcome Center, 166 Holly Smith Dr. Mcdonough Ga. 30253. 770-957-6601.

Parent/Guardian’s Photo Identification

Proof of Legal Guardianship (if applicable):

• This is required if the student is being enrolled by someone other than the birth parent listed on the birth certificate.

 

Copy of the child’s proof of age:

• Birth certificate    

Social Security Number (USED FOR FEDERAL REPORTING ONLY):

• Copy of student’s social security card (waiver option)

 

Transfer Students must provide the following information:

• Proof of grade placement
• Final report card from the previous school
• Name and address of the previous school

Immunization (state required):

• Georgia Certificate of Immunization/ Form 3231 must be current

 

Entering 6th-grade requirements:

All 6th-grade students must have updated immunization records before entering middle school, please check with your healthcare provider to make sure that your child’s immunizations are correct and updated for middle school enrollment.

 

Eye, Ear Dental Screening:

Copy of Georgia Form 3300
